FREE

Step 2 of 6

Let's start with your contact info.

Just 3 fields. This lets us reach you if anything comes up — and it's how you'll receive your competing quotes.

Your contact info is never shared with agencies until you personally select a winner. Zero unsolicited calls.
Only given to the agency you choose.
Only given to the agency you choose.
Step 3 of 6

Tell us about your fleet.

Every driver and truck. This is the core of your underwriting profile — accuracy here gets you the best quotes.

Drivers

Add every driver who operates your trucks. Driver history is the #1 factor agencies use to price your policy.

Power Units (Trucks)

Add each truck you want insured. VIN is on your dashboard, door frame, or title. Use NADA Guides or Truck Paper for market value.

Step 4 of 6

How do you operate?

These questions come directly from the ACORD 132 trucking underwriting form. Your answers determine coverage eligibility and pricing.

Cargo & Revenue
Pick the closest match. If you use load boards and haul whatever's available, select Dry Van — General Freight.
Do you ever haul pharmaceuticals, alcohol, tobacco, electronics, or other high-value goods? These require special endorsements. Most load board carriers answer No.
What is the most a single load you haul could be worth? If you use load boards and don't know, select the first option.
All trucks combined per year. 1 truck at 2,500/week = ~130,000/yr.
Your total trucking income per year. New operators — select the first option. This is used to calculate your liability rating.

Operating Details
How far from your home base do you typically operate?
Number of trailers you own. Enter 0 if you use drop trailers only.
All states you regularly run freight in. Affects zone rating and required state filings.
Where your trucks are parked when not in use.

Underwriting Questions

Standard ACORD 132 questions. Answer honestly — agencies verify through FMCSA and MVR records.

Do you haul to or from Canada or Mexico?Triggers additional coverage requirements and endorsements.
Do you haul hazardous materials (HazMat)?Requires H or X CDL endorsement and special FMCSA filings.
Do you haul target commodities?Electronics, pharmaceuticals, liquor, meat, seafood, or other high-theft/high-value goods.
Do other truckers operate under your DOT permit?Owner-operators or leased drivers dispatched under your authority.
Do you hire or lease trucks / equipment from others?Non-owned vehicle liability coverage may be required.
Do you have a formal vehicle maintenance program?Documented maintenance schedule can improve your rate with some carriers.
Are all driver DOT medical physicals current?Required by FMCSA. Lapsed physicals affect insurability with most carriers.
Do you use telematics, ELD data, or a dash cam?Technology can reduce premiums with several commercial trucking carriers.

Coverage Requested
$1,000,000 CSL is the FMCSA minimum for interstate carriers. Required to activate MC authority.
Covers freight if damaged, lost, or stolen. Required by most freight brokers.
Covers your truck in a crash. Required by lender if financed.
Select all that apply. Agencies will include these in their competing bids.
General Liability
Non-Trucking Liability
Occupational Accident
Trailer Interchange
Bobtail
Must align with MC authority filing if new operator.
Annual = lowest total cost. Premium finance = monthly loan payments.
Step 5 of 6

Almost done — insurance history.

Why this matters: Prior insurance history helps agencies offer better rates. New operators just answer "New operator" where asked. This is the last information step before your profile goes live to agencies.
Prior Insurance

If you've never had commercial trucking insurance, select "New operator" in the fields below.

Name of your previous insurance company.
What you paid last policy year. Enter N/A if new.
When your last policy expired.
On your declarations page. Enter N/A if new.

Claims History (Last 5 Years)

List any insurance claims in the last 5 years. New operators enter none. Agencies verify through loss runs.

Any claims filed in the last 5 years?

Loss Runs

Loss runs are official claims documents from your prior insurer. The winning agency may request them at binding.

Yes — I have them
N/A — New operator
No — I'll request from prior carrier

Background

Standard ACORD 125 questions required by all commercial carriers.

Has any policy been cancelled or non-renewed in the last 3 years? For any reason including non-payment.
Has the business or any owner filed for bankruptcy in the last 5 years?
Has any owner been convicted of fraud, bribery, or arson in the last 5 years?
Used by the winning agency for policy binding. Protected — never shown to competing agencies.
Step 6 of 6

One last step — authorize your driving record.

Agencies need your Motor Vehicle Record to give you a firm, bindable quote. We pull it once, securely, with your permission.

Must match your CDL exactly — used to pull your driving record.
Agencies see only your age range — never your actual birthday.
Encrypted. Never shown to any agency. Only used to pull MVR.
